Welcome to the first step of your journey with us.

To ensure we provide the best possible support for you and your family, we’ve gathered all of our service intake forms in one convenient place. We use these forms to understand your child’s unique story and your hopes for therapy before we begin.

How to get started:

1. Select the Relevant Form:
Choose the intake form below that matches the service you are seeking (e.g., Individual Therapy, Group Programs, or specific Assessments).

2. Complete the Application:
Fill out the details as thoroughly as possible. This helps our clinicians understand your child’s history and current needs.

3. Administration Review:
Once submitted, our administration team receives your form and adds your application to our waitlist.

4. Waitlist Monitoring:
When a place becomes available, we review our waitlist to find suitable candidates based when the form was submitted, clinical needs and, importantly, your provided availability.

5. Connecting with You:
Our administration team will reach out to you directly to discuss what is available. If the day and time are suitable for your family, we will then move forward with the next steps and schedule your parent-only initial session.
